Recently, the largest study on the prevalence of chronic diseases in humans, has reported that severe periodontitis was the sixth most common condition in the world, with an overall prevalence of 11.2%, approximately 700 million people. Other global reviews have concluded that moderate forms of periodontitis affect an even higher percentage of adults. In the United States, a large national study conducted between 2010 and 2012 found that more than 46% of the sample studied by adults over the age of 30, representative of 64.7 million people, suffers from periodontitis, spreading 8.7% mild, 30.0% moderate and 8.9% severe.

National studies have been conducted in Spain every five years since 2000, using the methodology advocated by the World Health Organization (WHO). To date, however, no surveys have been conducted aimed specifically at the oral health of the occupied population. The overall objective of this study was to assess the prevalence and severity of periodontal diseases in a representative sample of the working population in Spain, as part of a broader survey aimed at studying their oral state of health, as well as the relationship between the periodontal situation and its relationship with cardiovascular risk factors.

This oral epidemiological study was conducted using randomized sampling of workers, based on age, gender and geographical area (Center, Northeast, Northwest and South of Spain). The workers were consulted to perform the oral examination on the occasion of the visit for their annual medical check-up, leaving the sample for periodontal analysis composed of 5,130 individuals (Figure 1). Sociodemographic and habit variables were obtained from medical recognition and a completed questionnaire for this purpose. Five age strata were defined: under 25 years, 25-34 years, 35-44 years, 45-54 years and 55 years or older. By occupation they were grouped into manual workers (blue collar) and non-manual workers (white collar). The smoking habit was categorized into: non-smokers, ex-smokers, smokers of 10 cigarettes a day and smokers of > 10 cigarettes daily. The level of education was divided into three groups: low (primary), middle (secondary) and high (university). Finally, income levels (based on household monthly net income) were categorized into three levels: EUR 1,200, 1,200-3,600, and > EUR 3,600/month, respectively.

Periodontal clinical records were conducted using the Community Periodontal Index (CPI), and Insertion Loss (IP), following WHO criteria.

The results of this study show that the overall percentage of healthy subjects was totally only 5.5% (7.4% in women, 4.2% in men); the overall percentage of subjects with periodontal bags (periodontitis) was 37.3% (42.4% men, 30.0% women), significantly increasing with age, reaching 64.4% in subjects aged 55 years or older(Figure 2). In addition, periodontitis was more prevalent in subjects with manual occupation, primary studies, net monthly income of 1,200 euros/month, ex-smokers, and smokers of > 10 cigarettes per day.

When looking at which factors influenced the prevalence of periodontitis the most, separately for men and women, age was observed to be the most significant factor associated with the worst periodontal situation for both men and women. Referring to subjects who had never smoked, ex-smokers, smokers of 10 cigarettes/day and smokers of > 10 cigarettes/day predicted significantly a worse periodontal condition, both in men and women.

By comparing the periodontal situation of young adults (35-44 years) with previous national studies, the prevalence of periodontitis (29.2%) was higher than that of the 2000 and 2005 national surveys (21.4% and 21.5%, respectively), and significantly higher than reported in the 2010 study (11.0%)(Figure 3).

The findings of this study, which has evaluated a representative sample of the working population in Spain, showed that the worst periodontal situation was age-related (worsening from age 45), sex (worst in men), and was also related to smoking habits. The periodontal status in the young adult group was worse than reported in previous national studies conducted in Spain.

All of the above suggests that the situation has not improved over the years due, and that additional efforts should be made to increase oral health care, and specifically in prevention and basic treatment at the periodontal level. A complementary strategy could be that developed in this study, increasing oral evaluation and preventive measures in general health checks on workers. Such recognitions may offer the additional advantage of examining individuals who do not usually go to dental appointments, many of them thinking that their periodontal situation is correct because of ignorance and information errors.

DR.
