Ridge morphology compatible with a transcrestal approach.<\/p>\n<p><strong>Therapeutic objective<\/strong><br \/>To obtain sufficient vertical bone regeneration beneath the sinus floor to allow for a delayed implant placement in site 2.6, targeting a final bone height \u2265 8\u201310 mm while preserving the integrity of the Schneiderian membrane.<\/p>\n<p><strong>Treatment planning<\/strong><\/p>\n<ul>\n<li>Transcrestal sinus floor elevation selected due to narrow sinus morphology.<\/li>\n<li>Crestal access performed with piezoelectric inserts to minimize perforation risk<\/li>\n<li>Use of a collagenated injectable grafting material that simultaneously lifts the sinus membrane and fills the newly created subantral space<\/li>\n<\/ul>\n<p><strong>Surgical procedure<\/strong><\/p>\n<ol>\n<li>Local anesthesia and minimally invasive flap elevation (or flapless technique when anatomy permits).<\/li>\n<li>Crestal osteotomy at site 2.6 using a piezoelectric tip until contact with the sinus membrane is achieved.<\/li>\n<li>Membrane integrity verification(Valsalva maneuver).<\/li>\n<li><strong>Injection of a collagenated biomaterial<\/strong>\u00a0to obtain a dome-shaped elevation of the sinus membrane.<\/li>\n<li>Suturing; postoperative pharmacological support according to standard protocols.<\/li>\n<\/ol>\n<p><strong>Radiographic and clinical follow-up<\/strong><\/p>\n<ul>\n<li>Radiographic evaluations (periapical radiographs) at\u00a0<strong>2, 4, and 6 months<\/strong>\u00a0to assess graft stability, volumetric maintenance, and mineralization.<\/li>\n<\/ul>\n<p><strong>Histological sampling<\/strong>\u00a0of the regenerated tissue at the time of implant placement (6 months) shows overall well-structured, organized bone tissue with good vascularization and no fibrosis or significant inflammatory signs.
